At this time Dr. Silvertooth does not accept any insurance plans, but will provide patients with a claims form.

 

Patient fees are determined by the visit type, and are at a flat rate regardless of which services are provided.  Visit types include the following:

New Patient Evaluation
Iincludes review of symptoms, prior treatments and old records, physical examination (when needed), discussion of recommended treatment(s), further evaluations and/or referrals.

30 minute visits
May include medication management, physical exam, and/or psychotherapy, depending on individual patient needs.

Longer appointment times are available as needed.
